Chronic Clinic Forms

Each chart contains documentation for detainees that need to be seen in Chronic Clinic Forms. The Chronic Clinic Forms has a workflow associated with its creation. When a nurse begins a new Chronic Clinic form, he/she will send the document to the Practitioner’s Task List to be completed during the patient encounter.

A screening tool known as AIMS (Assessment for Involuntary Movement Scale) is available to be completed for those detainee’s on psychotropic medications. Also included is an area to document lab results pertaining to the detainee’s specific problem or diagnosis.
